Job Summary:
Unicity Homecare is seeking an experienced, compassionate, and driven Home Care Field Nurse Supervisor to join our team in New Jersey. In this role, you will be responsible for managing and overseeing Home Health Aides, ensuring the delivery of high-quality client care and maintaining compliance with all regulatory standards. This is a full-time, salaried position in the healthcare industry, with a competitive salary and benefits package.

Compensation & Benefits:
The Home Care Field Nurse Supervisor position at Unicity Homecare offers a highly competitive salary of $95,000 to $105,000 per year, paid biweekly. Responsibilities:
• Manage and supervise Home Health Aides, providing support, guidance, and mentorship to ensure quality patient care and compliance with company policies and procedures
• Conduct regular performance evaluations of Home Health Aides and provide feedback for professional development
• Oversee and coordinate the scheduling, assignment, and completion of patient care services
• Collaborate with other healthcare professionals, such as physicians, therapists, and social workers, to develop individualized treatment plans for clients
• Monitor and review client charts to ensure documentation and care plans align with regulatory requirements and best practices
• Participate in orientation and training of new nurses to ensure they understand and follow company policies and procedures
• Conduct regular nursing visits and make recommendations for improvement
• Act as a liaison between clients, families, and healthcare providers to ensure overall satisfaction and quality of care
• Develop and maintain relationships with community resources and referral sources
• Stay informed about changes and updates in the healthcare industry, specifically in home care, and implement necessary changes to maintain compliance

Requirements:
• Valid RN license in the state of New Jersey
• Minimum of 3 years of experience in home care or related healthcare setting
• Knowledge of applicable state and federal regulations for home care
• Excellent communication, leadership, and problem-solving skills
• Ability to work independently and make critical decisions
• Proficient in electronic medical records and Microsoft Office
• Valid driver’s license and reliable transportation
