POSITION SUMMARY

Responsible for assisting in the coordination of the Nursing Service performance improvement activities including, but not limited to, data abstraction for Centers for Medicare and Medicaid Services (CMS) and/or California Department of Public Health (CDPH) surveys, Joint Commission Evidence of Standard Compliance (ESC) audits, NDNQI, performance improvement projects and other regulatory quality outcomes studies. Primarily responsible for coordination of data with variances and data reporting to the Assistant Chief Nursing Officer, Nursing Directors, and Director of Quality and Patient Safety.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Conducts analysis of patient medical records and provides data and information to meet regulatory requirements.
  • Conducts timely and accurate chart abstractions following current abstraction guidelines for CMS, Joint Commission, and national data registries (i.e. NDNQI.
  • Completes abstraction and data validation with minimal supervision and direction and with accuracy.
  • Effectively utilizes online data abstraction tools, reports, resources and internal databases.
  • Assists in finalizing data for submission to external regulatory agencies.
  • Submits data to external entities (i.e. NDNQI) timely.
  • Conducts daily and concurrent chart audits to ensure compliance with internal and external regulatory standards.
  • Tracks outliers and provides data reports and other information related to specific metrics to facilitate analysis and follow-up for the purpose of improving core measure compliance.
  • Performs concurrent and retrospective case reviews based on generic screens, clinical indicators, focused studies, physician or outcome issues and occurrence reporting.
  • Prepare reports and dashboards for committee meetings using the most appropriate tool to display data.
  • Assists nursing leaders by providing feedback directly to the staff and uses the data for continuous improvement and learning.

Job Requirements

POSITION REQUIREMENTS

A. Education

  • A Bachelor’s degree in Nursing required. Master’s degree preferred.

B. Qualifications/Experience

  • CA RN license required.
  • Education and experience in statistical concepts, data management, variance tracking and analysis preferred.
  • 1-2 years’ experience in chart auditing, performance improvement and/or data collection preferred.
  • Must be flexible to handle a variable workload, deadlines, and priority changes. 
  • Must be able to work effectively with management, administration, medical staff, nursing, and ancillary departments. 
  • Excellent computer skills are required including proficiency in (MS Office (Word, Excel, and Access).
  • Detail oriented organizational skills. 
  • Must be able to handle multiple cases, directions and be able to follow-through.
  • Good communication skills both verbal and written. 
  • Experience with Medical Staff communication preferred.
  • Excellent interpersonal relationship skills with exceptional professional work ethics.

Special Skills/Knowledge 

  • Excellent communication skills and evidence of collaborative practice. 
  • Knowledge of the CMS, CDPH, NDNQI and other processes as they pertain to the organization.
  • Proficient with computer software utilized to monitor projects or/program in place for Core Measures.
  • Proficient in email, Excel and internet programs. 
  • Ability to collaborate in a team environment.
  • Demonstrated ability to utilize necessary tools to monitor and trend selected areas of improvement.
