Position Summary:

The Inpatient Pharmacist is responsible for ensuring the safe, effective, and rational use of medications within the hospital setting. This role provides clinical and distributive pharmacy services to optimize therapeutic outcomes, minimize adverse effects, and promote cost-effective medication use. The Inpatient Pharmacist collaborates closely with physicians, nurses, and other healthcare professionals to deliver high-quality patient-centered care.

Key Responsibilities:

Clinical & Patient Care:

  • Review and verify physician medication orders for appropriateness, accuracy, dosage, and potential interactions.
  • Monitor patient medication regimens for therapeutic effectiveness, adverse reactions, and drug interactions.
  • Participate in interdisciplinary patient care rounds to provide pharmacotherapy expertise.
  • Adjust dosing for renal/hepatic impairment and therapeutic drug monitoring (e.g., vancomycin, aminoglycosides, anticoagulants).
  • Support and promote antimicrobial stewardship initiatives.
  • Provide patient counseling when appropriate (e.g., prior to discharge or high-risk medications).

Dispensing & Distribution:

  • Accurately prepare, compound, and dispense medications according to established policies and legal requirements.
  • Supervise and verify the work of pharmacy technicians to ensure safe and accurate medication preparation.
  • Maintain controlled substance accountability in compliance with DEA and facility regulations.
  • Ensure timely delivery of medications to patient care areas.

Documentation & Compliance:

  • Document clinical interventions, medication histories, and patient education in the Electronic Health Record (EHR).
  • Maintain current knowledge of hospital formulary, medication-use policies, and Joint Commission standards.
  • Ensure compliance with all state, federal, and institutional pharmacy laws and regulations.

Collaboration & Education:

  • Serve as a resource for drug information and clinical guidance to healthcare staff.
  • Participate in staff education, in-services, and pharmacy student/preceptor programs as assigned.
  • Contribute to quality improvement and medication safety initiatives.
Qualifications:

Education:

  • Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy from an accredited college or university required.

Licensure/Certification:

  • Current and active Pharmacist license in [State of Practice] required.
  • Basic Life Support (BLS) certification preferred.
  • Advanced certifications (e.g., BCPS, BCOP) a plus.

Experience:

  • Minimum of 2 years of recent inpatient or hospital pharmacy experience required.
  • Experience in an acute-care setting strongly preferred.
  • PGY1 or PGY2 Pharmacy Residency preferred but not required.

Skills & Competencies:

  • Strong clinical knowledge in pharmacotherapy, dosing, and disease management.
  • Excellent analytical, problem-solving, and decision-making skills.
  • Proficient with hospital pharmacy systems (e.g., Epic, Cerner, Meditech) and automated dispensing cabinets (e.g., Pyxis, Omnicell).
  • Effective communication and teamwork skills.
  • Attention to detail and commitment to patient safety.
